OIT Help Desk Hardware Reservation Form

This form is used to request a reservation for hardware.

Only University of Maryland, College Park faculty, staff, and teaching assistants are eligible to rent hardware.

Please note that this request does not ensure a reservation until your request is confirmed by a staff member at the Office of Information Technology (OIT) Help Desk.

Before submitting this request, please make sure that you read and understand the following conditions:

  1. Only Faculty, Staff, and Teaching Assistants (TA) can borrow hardware.
  2. Reservations can be made by people other than the Faculty/Staff/TA member (for example, by an administrative assistant) , but the person who wishes to use the hardware (the borrower) must come and checkout the reserved item(s). Furthermore, at that time a current factulty/staff ID card must be presented by the borrower.
  3. If the borrower cannot checkout the reserved item(s), then the Hardware Permission Form must be completed and presented at that time.
  4. On the first day of the loan, patrons must pay the fee by cash, check, or Internal Service Request (ISR) if they are paying with their FRS account. We do not accept credit cards.
  5. Rental fee applies only for non-holiday weekdays.
  6. There is a one day minimum for rentals.
